My name is Lisa Johnson, and I am the owner and provider of The Vineyard AFH LLC. We have medicaid, ECS,SBS or private paid, private bedroom, room available in our home, which specializes in providing compassionate, one-on-one care for residents with beginning stages of dementia (non-exit seeking) and low to mild mental health needs.
At The Vineyard, we pride ourselves on creating a peaceful, supportive environment where our residents feel at home. Here’s what we offer:
• Specialized Care: Services include urostomy bag care, tube feeding, diabetic needs, mental health, with specialty programs that clients may qualify for we offer a therapist and psychological nurses on staff, managed with the assistance of a Nurse Delegator.
• High-Quality Amenities: A built-in Hoyer lift (not a two-person assist) is available for residents who need it.
• Personalized Attention: With a maximum of six clients at a time and two caregivers often on-site, we ensure individualized care.
• Comprehensive Support: We accommodate Medicaid and private-pay clients and are committed to supporting residents through end-of-life care.
• Stable, Caring Staff: Half of my staff have been with me for over three years, and we haven’t had any turnover for the past year and a half.
We also encourage community engagement and activities to enrich the lives of our residents. Many of the ladies who live here find our quiet, serene home setting to be a source of great comfort and joy.

Lisa and her team treated my aunt feel like she was part of their family. They took excellent care of her and helped her maintain a good quality of life. Her days there included healthy meals prepared daily, a spotless facility with expansive yard and a porch for her to watch the neighborhood, a bouncing Pomeranian named Sadie, and trips for hair appointments and other excursions. There were also holiday parties to attend, which were family affairs with copious amounts of glittering decorations, sugary treats, and laughter. I would highly recommend the Vineyard. Darla was treated with the care and dignity she deserved. Although she will be missed, I am happy we chose them to help brighten her remaining years.
