Welcome to Legacy AFH! We specialize in providing the highest standards of care to Ventilator management & Tracheotomy care patients. We are an RN Owned and Operated Home with a Nurse and CNA on staff 24/7. We have in-house physician and lab visits, personalized resident plans, comprehensive computerized medication management system and so much more.
