The same exceptional care from the same exceptional team at Family Home Health, now with a new name. Family Home Health has retained its ownership, management, and staff while re-introducing itself as Craft Health. We are a family owned and operated home health and hospice agency based out of Boulder. Our medical home health services include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, CNA and Caregiver CNA program. Our non-medical home health services include personal care, homemaker, respite care and In Home Support Services (IHSS). Our hospice services include skilled nursing, CNA, physician, chaplain, medical social work, bereavement counseling, and volunteer services. Our mission is to provide the highest level of personalized care and services specifically tailored to the needs of our patients and participants. Our vision is to create a lasting legacy, that if we should ever happen to cease to exist, the community would experience a great loss.
