About

Sunrise Health Services

Sunrise Health Services is a Medicare-certified home health agency serving Mesa, Arizona and surrounding areas. A Medicare-certified home health agency provides skilled nursing services, physical therapy, occupational therapy, speech-language pathology, medical social services, and home health aide services to patients in their homes.

Medicare covers home health services for patients who are homebound and require skilled nursing care or therapy services on an intermittent basis. A physician must certify that the patient needs home health care and establish a plan of care.

Home health agencies are certified by the Centers for Medicare & Medicaid Services (CMS) and must meet federal Conditions of Participation. These requirements ensure that agencies provide quality care, maintain proper documentation, and protect patient rights.

Medicare beneficiaries pay no cost for covered home health services when provided by a Medicare-certified agency. For more information about home health benefits, visit Medicare.gov. Compare home health agency ratings at Medicare Care Compare.

Quality ratings and agency data are updated periodically by CMS. We recommend verifying current information at Medicare Care Compare.

I am so grateful for the care that Sunrise has provided for my grandmother. From the very beginning, the staff was compassionate, attentive, and professional. Every nurse and therapist who came into our home treated her with dignity and respect, and you could tell they genuinely cared about her well-being. They went above and beyond to make sure she was comfortable, safe, and supported every day. Their communication with our family was excellent, always keeping us updated and making us feel at ease knowing she was in such good hands. What stood out most was the kindness they showed—not just to my grandmother, but to all of us. It truly felt like we had an extended family caring for her. I highly recommend Sunrise to anyone in need of home health services. Their compassion and dedication made such a difference during a difficult time, and we will always be thankful for the love and care they gave my grandmother.

Sunrise Home Health took care of my grandma’s physical therapy after her surgery. From the very beginning, the team was professional and genuinely attentive to her needs. The therapist was not only skilled, but also patient and encouraging, which made a huge difference in her recovery. It gave our family so much peace of mind knowing she was in such good hands. I’m really grateful for the care they provided and would definitely recommend them to anyone!

My mother recently received Services from Sunrise Health services having had surgery and I have to say what amazing care they provided to her and getting her back to regaining het mobility. We were very happy with the kind staff throughout her recovery. The Medical social worker also helped us with many community resources we didnt even know existed to us. I would highly recommend them to anyone seeking great care for their family member.

Sunrise Home Health has been an absolute blessing for my family. From the very first visit, their team showed incredible professionalism, compassion, and genuine care. They don’t just provide medical support—they bring peace of mind, dignity, and warmth into the home. Every nurse, therapist, and staff member we’ve worked with has gone above and beyond to make sure our needs were met and that we felt supported every step of the way. They take the time to listen, explain, and tailor care to each individual’s situation. It’s clear they truly care about the people they serve, not just the job. What really stands out is the consistency of their quality. Whether it’s skilled nursing, therapy, or just the extra touches of kindness, Sunrise Home Health sets the bar high. It feels like having a team of extended family members looking out for us. I can’t recommend Sunrise Home Health enough to anyone in need of home health care in Mesa. They are reliable, compassionate, and outstanding in every way. Truly a 5-star experience!

Not a good fit for my father in law who had alzheimers. The last CNA was good (hence the 2 vs 1 star rating) but generally the care provided was minimal and inconsistent. Would not recommend unless you can spend time staying on top of this company to make sure the care is provided consistently. Throughout the roughly 8 weeks of care, I tried to escalate issues immediatley and called the office at least three times. Some adjustments were made. I did have one person in the office address seem to care and address some issues at the end which was appreciated. If you want to keep reading here are some specific issues 1. Even though it was explicitly requested and agreed to, the company did not provide the same caregivers or the same schedule consistently which is very disruptive for a person with dementia to have different people/schedule/inconsistency of care. I understand if there was a sub because someone had time off but this was a consistent issue and caused a lot of confusion and stress. 2. Scheduling for care givers was sometimes done within hours of when they wanted to come or they would show up without an appointment. Some caregivers would find him at his facility and perform services but on at least 2 occasions they counted it as missed appointment even though no one knew they were coming. Caregivers would just not come per the frequency agreed to or they would stack multiple caregivers on a day. They requested at least once to have all 4 caregivers come on one day. All of this defeated the point of having people come throughout the week for care. 3. The company had clear direction to have any/all paperwork go through the POA as my father in law is not cognitively capable of signing anything. The biggest issue was that the nurse had my father in law sign his first name on his discharge papers and then gave the copy to him. He has no clue what he is signing and of course the paper was lost. The nurse did send a copy via text after he told us what he had done and the POA requested a copy. The POA was present for the final visit and could have signed but the nurse had my father in law sign a week prior to the last visit. 4. Caregivers were told to not provide any medical information or care with anyone but the family but this was disregarded and care was provided in the presense of other residents as well and at least one other resident was given information about his care.
