Services Provided: • Personal Assistance Services Agency: Host home Individual Residential Services &Supports (IRSS), Supported Community Connections (SCC), Personal Care & Respite, Homemaker, Mentorship. • Skilled Nursing: Skilled assessment and teaching. Post-surgical care, disease management, and medication management. IV therapy, wound care ostomy care, glucose monitoring, lab work. Other services as needed. • Physical Therapy: Conditioning and restoration of muscle strength. Balance, ambulation, and range of motion. Wound care. • Occupational Therapy: Skills necessary for skilled care and activities of daily living. • Speech Therapy: Speech recovery or alternate ways to express needs and feelings. • Certified Nurse Aides: Personal care and hygiene. Light homemaking services. • Medical Social worker: Assistance with coping with the impact of illness and disability.
