B & O SERVICES INCOPORATED JOB DESCRIPTION CLIENT SUPPORT WORKER Reports To: Client Support Worker Team Leader SUMMARY OF ROLE: To provide support services to adults with a learning disability and fosters self-awareness, personal growth and helps to foster decision-making and life style choices. Shift Duties and Responsibilities: 1. Serve breakfast. lunch, and/or dinner 2. Assist/Supervises showers and dressing residents 3. Ensure residents’ rooms are cleaned and vacuumed. 4. Clean kitchen area. Wash stove eyes after cooking and clean stove/microwave if used. 5. Clean master and hall bathrooms. 6. Prepare lunch and dinner. 7. Make weekly menu. 8. Shop for groceries and household items. 9. Clean residence totally (all rooms and baths). 10. Possible doctors’ appointments for residents. 11. Transport residents to day programs (must be licensed and insured to drive residents. 12. Pick residents up from day program. 13. Pick up prescriptions. 14. Place all folded washed clothing in individual residents’ room in the appropriate area. 15. Ensure residents’ clothing in closets and drawers are neatly arranged. 16. Serve snack when residents return from day program. 17. Monitor medication being taken by residents and sign medication log. 18. Assist residents with any prescribed goals identified in their treatment plan. 19. Document residents’ progress hourly in progress notes (must be signed and dated before departure from shift and it must be done during working hours. (No Overtime). 20. Perform all duties identified in your job description and those in which will ensure a safe and clean environment for the residents. All dirty diapers are to be put in a small plastic bag. Tie it! Then take it outside and put it in the largest garbage can. Shift Responsibilities con't: 1. Wash and dry client clothing. 2. Assist residents in setting dinner table. 3. Prepare/warm evening meals. 4. Serve evening meals to residents. 5. Assist residents in performing daily life skills, i. e. setting dinner table, clearing dinner table, assist them in dish washing and putting utensils in the proper place in the kitchen. 6. Serve snack as scheduled. 7. Assist residents with any prescribed goals identified in their treatment plan. 8. Monitor medication being taken by residents and sign medication log. 9. Provide activities for residents (games, TV, computer, basketball, etc.). (TVs are to be shut off at 10 PM). 10. Assist residents in taking showers and preparing for bed. 11. Remove residents’ dirty clothes basket and place by the residents’ door before the residents retire for bed. 12. Monitor all residents once they are in bed. 13. Document residents’ progress hourly in progress notes (must be signed and dated before departure from shift and must be done during work hours. NO Overtime). 14. Perform all duties in your job description and those in which will ensure a safe and clean environment for the residents.
