Role: Hospice Case Manager Job Location: Los Angeles, Orange County, Riverside, Kern County, Fresno County. Position: Freelance Salary Range: Depends on Experience SUMMARY We are looking for a Hospice Case Manager to join our team at Vela Hospice Care, Inc. The Case Manager will responsible for overseeing the care of patients who are nearing the end of their lives. The Case Manager will work with a team of medical professionals to ensure that these individuals receive high-quality palliative care, including physical, emotional, and spiritual support. Hospice case manager also help families navigate this difficult time by providing them with resources and information about local services. Responsibilities: Assessing patients’ social and medical history to develop plans of care that meet their needs. Meeting regularly with patients and their families to monitor the patients’ physical, emotional, and spiritual needs. Coordinating care with other medical providers and social services agencies in order to meet patients’ needs. Monitor and direction all aspects of patient care, including Hospice/ home health services, physician appointments, and community resources. Providing counseling and support to patients and families during difficult times in their lives. Maintain accurate and up-to-date documentation in the patient’s medical record. Coordinating care with other medical providers and social services agencies in order to meet patients’ needs. Coordinating with insurance companies to ensure that patients receive adequate coverage for their treatment plans. Recommending appropriate treatments and equipment for patients based on their condition and available resources Perform other duties as assigned.
