Care Partner

• Performs personal care tasks o hygiene, ambulation, eating, dressing, toileting, and shaving • Provides transfers assistance by moving members in and out of beds, baths, wheelchairs, or cars • Maintains a specialized approach to responsibilities such as uniforms, start times and absenteeism • Takes and record vital signs and weights • Help with sanitation and housekeeping duties of member’s room, bathroom, and common areas • Perform general household tasks including laundry, shopping, errands, and transportation • Implements and support member-care plans • Plan, prepare and monitor meals with members • Provides assistance with walking and light exercises • Encourages members to engage in both physical and mental exercises • Observes changes in behavior or medical condition and report changes immediately
