1. Follow the six ‘rights” of medication administration (right resident, right drug, right dosage, right time, right route, right record/documentation) each and every time assistance with medication occurs. Check each of the six “rights” of medication assistance three different times: • When the medication is taken out of the medication cart or storage • Immediately before assisting with/ medication • Immediately after assisting with the medication 2. Assist with medications for resident according to their Medication Observation Record (MOR), if no errors are noted, uphold the residence’s philosophy, and protect resident privacy. 3. Document clearly and accurately on the Medication Observation Record (MOR) after a medication is taken/given, refused, held, discontinued, changed, disposed of, or returned, 4. Maintain a neat, clean, organized medication cart/cabinet, and restock the cart/cabinet as needed. Medication Assistant Job Description Resident Services 1. Provide quality care and assistance to residents in accordance with the Oakmonte Village philosophy, policy and procedure, and resident rights. 2. Assist residents in meeting their physical, emotional, spiritual, and medical needs to achieve and maintain their highest level of wellness. 3. Promote and protect the residents’ rights to safe, prompt, and confidential treatment. 4. Answer and respond immediately to resident calls. 5. Respect the residents’ right to refuse medication/treatment. 6. Protect the residents’ safety with medications. 7. Explain to the resident what medication is being given and why, prior to assisting with the medication. 8. Observe the resident for any adverse reaction to a medication and report it to your Nurse or supervisor immediately. 9. Maintain confidentiality of residents’ records. 10. Control the spread of infection by following standard precautions. 11. Dispose of infectious and potentially infectious waste according to procedure, and OSHA regulations. 12. Establish and maintain a good relationship with residents and their families. 13. Respond to resident emergencies following proper procedure. 14. Carry out all duties properly and effectively as assigned by your supervisor. Communications 1. Attend all regular staff meetings and required training sessions. 2. Effectively communicate information pertaining to the residents to other team members according to residence policies and procedures. 3. Effectively communicate to the Nurse any changes noted in a resident’s condition or behavior, adverse reaction to a medication, any resident concerns, complaints, refusal of a medication, any errors noted on the Medication Observation Record (MOR). 4. Report any and all medication errors honestly and immediately. Medication Assistant Job Description 5. Seek assistance and advice from your supervisor immediately if you have any doubts or questions when assisting with/medications before you perform the assigned task. 6. Respond to on-the-job injuries in accordance with the residence policies and procedures. 7. Notify your supervisor of any concerns or questions presented by the residents’ family members. Physical Job Requirements To fulfill job requirements, employee must be able to perform key physical task for extended periods of time. When supporting, lifting or transferring residents, proper body mechanics and safety measures as prescribed by the Oakmonte Village must be used. In a typical eight- hour day, employee will: • Stand/walk up to six hours a day • Sit up to two hours a day • Frequently support up to 50 pounds • Occasionally lift/carry up to 50 pounds • Frequently kneel, bend, and reach • Secure proper assistance for transferring and lifting of residents as needed
