Licensed Practical Nurse (LPN) Job Description
We are seeking LPNs in the greater Augusta, GA area who have experience providing Private Duty Nursing Care to patients who are medically fragile with complex care needs as a result of a chronic condition(s) / disease(s).
Nurses providing this very specialized level of in-home care will do so one-on-one with one of our homecare patients, for anywhere from 8 to 12 hours per shift. As apart of their care needs, our patients all typically have one or more of the following assistive medical devices or treatment regimes, including; feeding tubes, tracheostomy or other respiratory management needs, including and up to home ventilators.

JOB RESPONSIBILITIES:
Administer nursing care to ill, injured, or disabled patients in line with Physician Orders
Delivering care consistent with patient Plans of Care Service Plans
Monitor and report changes in patient symptoms or behavior to appropriate supervisor and/or patient physician
Communicate with collaborating physicians or specialists regarding patient care
Educate patients about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Document care provided in real-time, maintaining accurate patient medical records
Provide education and support to patients and their family members on disease process & treatment
Strong problem solving and critical thinking skills, with ability to preform under possible emergent situations
Ability to thrive in a fast-paced environment
JOB REQUIREMENTS:
Active Georgia RN or LPN Nursing License
At least one (1) year of prior home health care experience
BLS CPR Certified
Prior experience in Chronic Disease Management, Preferred
Self-Starter, highly motivated and dedicated
Experience working with patients with feeding tube, tracheotomy, or ventilator A Plus
Copy of recent Physical, when applicable based upon state and federal regulations
Current TB screen results/Chest X-Ray
Reliable Transportation
Valid Driver’s License with Clean DMV Record, Required for Client Transportation Assignments
