Registered Nurse (RN) Job Description
DUTIES The Director of Nursing (DON), in accordance with policies and standards established by the Governing Body, and under the direct supervision of the Administrator, assists with planning, coordination, leading, and controlling and evaluation of home health services. RESPONSIBILTIES Working in close collaboration with the Program Director and Managing Director: Responsible for Oasis processing and overall coordination and scheduling of home care visits and supervision of clinical documents. Perform skilled nursing visit when appropriate and as needed. Develops short- and long-term plans and ensures the implementation to improve the delivery of the Home Healthcare programs and achieve the goals set by Solace Home healthcare Solutions, LLC. Ensures overall clinical and administration of Solace Home Healthcare Solutions, LLC. Manages the Home Healthcare Care clinical record or chart reviews. Experience working with administrative leaders to manage, clinical operations, and/or population health programs within a home health agency Develops, implements and ensures the ongoing execution of the expansion of the Solace Home Healthcare Solutions' programs; Determined medical necessity for skilled nurse and home health aide. Oversees and manages the full revenue cycle for the Home Healthcare programs; ensures the optimization of professional and technical reimbursement and collaborates closely with Solace Home healthcare Solutions, LLC billing efforts/compliance, and payer relations. Leads efforts to improve and expand the performance of the Home health aide programs; Develops and monitors key performance indicators to ensure that goals are met; such as referral processing, admission efficiency, and QAPI program to ensure timely and accurate data is available for key initiatives Serves as the liaison to primary care office, hospitals, and other partners across the community. QUALIFICATIONS Must be licensed in the state of Massachusetts as a Registered Nurse. 3-7 years experience preferably in home/community administration, and supervision experience. Must submit a background check and current CPR certifications Bachelor's preferred.
