Licensed Practical Nurse (LPN) Job Description
Education and Experience Required: Licensed Practical Nurse. Maintains a current/unencumbered LPN license in the state of Georgia. Minimum of 1 year of clinical experience in long term care, community based care or assisted living. Prior experience and training working with the disabled and/or youth population and their families faced with emotional and behavioral challenges. Minimum Physical Requirements: Must be able to go up and down stairs; bend and lift; and perform physical restraints. Position Summary: This position supports Lillie’s Healthcare Home Health Agency. The Licensed Practical Nurse (LPN) will be responsible for maintaining quality of care throughout the program, promoting the health and wellbeing of each client, and reporting observed changes in client condition or health to management team. Responsibilities: 1. Ensures quality client care is consistently delivered in accordance with all local, state and federal guidelines. 2. Records client’s medical history accurately. 3. Takes and records vital signs as necessary. 4. Assist clients in daily necessary activities. 5. Administer injections, prescribed medications, etc, clean and dress wounds and assist with other basic medical care tasks. 6. Collaborates and partners with management and direct care workers to promote quality care and independence for our clients. 7. Maintain, through communication and documentation, positive and collaborative relationships and continuity with other care providers. 8. Ensures compliance, efficiency and team’s understanding of policies and procedures. 9. Ensure that all client care plans have the documentation necessary to provide the proper type of care. 10. Participate in scheduled On-Call duties as needed. 11. Additional duties as assigned.
