RESPONSIBILITIES
-Provides safe and effective care for the client based on individualized Plan of Care in conjunction with Clinical Manager.
-Provides prompt communication of changes in patient medical status/emergencies to the team resulting in effective management of cases.
-Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and knows whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.
REQUIRED EXPERIENCE & EDUCATION
- LPN license in the state of Georgia.
-CPR
-Current TB Test
-Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
