Licensed Vocational Nurse (LVN) Job Description
Qualifications:
• California License as a Licensed Vocational Nurse (LVN)
• Current CPR Card
• At least 1 year of clinical experience in acute care/in- patient or outpatient facility, extended care or skilled nursing facility or home health experience.
• Must be a team player to ensure close, clear and timely communication with a multidisciplinary team.
• Must possess intermediate computer skills and be comfortable with electronic records keeping and updating.
Basic Function:
• Under the supervision of the Registered Nurse, is responsible for the provision of skilled nursing services to participants requiring medical care, including implementation of the medical/nursing and personal care section of the individual plan of care (IPC).
• Participates in the multidisciplinary team meetings.
Job Responsibilities:
• Assist the Registered Nurse in determining the needs for participant’s personal care, assistance with activities of daily living.
• Provide skilled nursing services as indicated in the medical/nursing & personal care section of the participant’s Individual plan of Care (IPC), which include but not limited to:
• Monitor and record vital signs.
• Monitor and record signs and symptoms of participant’s illness.
• Monitor, administer and record prescribed medications.
• Perform direct hands-on/skilled nursing services such as wound care, administration of parenteral medications, etc., as indicated in the treatment plan or as ordered by MD.
• Health education and counseling to participant’s and/or families as indicated in the Individual Plan of Care (IPC).
• Document all skilled nursing services provided to each participants using the required forms.
• Assist the Registered Nurse in providing the following services to the ADHC participants and/or families:
• Obtaining necessary medical services from providers other than the adult day health staff.
• Providing liaison to the participant’s personal physician.
• Providing supportive and restorative nursing as indicated in the treatment plan.
• In emergency situation, contacting 911 and communicating/ coordinating with the paramedic unit as the condition requires.
• Providing supervision to Program Aide who provides/assists participants with personal care, activities of daily living and maintenance therapy program.
Performs other duties as assigned by the Registered Nurse, Program Director, Administrator and/or members of Multidisciplinary Team.
