We are looking for a compassionate Caregiver/Personal Care Aide/Sitter/CNA'S to provide assistance in daily living in patients homes in accordance with an established care plan. Responsibilities: Provide home care services in clients' residence Perform/Assist with ADLs and IADLs May transport and accompany patients to doctors office or to hospital Medication Reminders Assist with clients personal care activities Monitor (vital signs, food and water intake, behavior and skin changes) and report on their condition Document task via smart phone Assist clients with mobility and physical therapies/exercises Engage clients with activities Provide companionship and basic emotional or psychological support Skills: Valid Drivers License with reliable transportation Familiarity with basic nutrition and personal hygiene standards Proficiency in English Caring and compassionate personality Experience with Alzheimer's/Dementia Current CPR/First Aid certificate Valid driver’s license High school diploma Current TB Successfully complete new-hire modules and quarterly modules
