Hospice RN Case Manager - Central Valley, Signing Bonus!

Elements of Elder Care is growing their nursing team in the Central Valley - we are looking for an experienced RN Case Manager to join our team with a $1000 Signing Bonus! Duties to include: Develop and maintain the patient plan of care Patient care - "Head-to-Toe" Assessments Bi-weekly Interdisciplinary Team Meetings (IDT) Education and emotional support for patient and families at end of life Travel to and from patient homes, facilities and the office as needed Constant communication with other members of your team Job Requirements: 1 year of hospice experience preferred, not required Must be willing to drive Must work with Integrity Strong work ethic and a self-starter Creativity with a willingness to collaborate Salary is negotiable and based on experience.
