Minimum of one year experience in any health care settings, preferably in home health or hospice
Strong comprehensive clinical assessments and reassessments skills
Must be proficient in documentation skills including development of patient specific care plan. OASIS Assessment experience is a plus.
Experience in use of Electronic Medical Record System.
Demonstrated competency in use of computer, basic function and basic knowledge of word program
Effective verbal and written communication skills
Ability to supervise CHHA/LVNs for oversight of care plan

Must have dependable transportation to perform home visits
Must be comfortable to deliver care in diverse community
