Licensed Vocational Nurse (LVN) Job Description
Mission de la Casa is an independently owned & operated SNF in San Jose. We have 163 licensed beds that include a 41-bed secured memory care unit. Responsibilities: - Admit and discharge patients. - Safeguard the resident’s privacy and property. - Follow safety policies in performing nursing care. - Following safety policies when administering medications or performing treatments. - Collects and records data necessary for quarterly resident assessment as coordinated by the MDS RN. - Knowledge of current wound and skin care management protocols. - Complete assessments, initiate evaluation, and resolve plan of care as appropriate. - Communicate with physician as indicated by resident status, needs, and assuring follow through. - Perform daily rounds to determine condition, comfort, and response to medications and treatments. - Administer medications as prescribed and implementing other nursing intervention as indicated by resident plan of care or as ordered by physician. - Initiate emergency measures according to facility’s policy and within standards of nursing practice. - Manage staff, oversee patient care, and ensuring adherence to established policies and procedures. - Ensuring provision of quality nursing care with Federal, State and Company requirements. Requirement: - Able to read and write English. - Good physical and mental health. - Be a caring person with good judgment skills. - Must be licensed as a Nurse, in good standing in the state of California. - Ability to apply basic principles of nursing care base on standards of nursing practice.
