Licensed Vocational Nurse (LVN) Job Description
Perpetual Care Hospice is seeking a Full Time compassionate and responsible LVN-Case Manager to join our team.
· The Case Manager coordinates and directs the clinical, operational, and program activities on a day-to-day basis to ensure delivery of quality patient care.
· Case Manager will work collaboratively with Physicians, Medical Directors, Nurses, Social Worker, Chaplain and Hospice aides as an integral member of the interdisciplinary team.
· Case Manager assume the overall responsibility for organizing patient care, develop the interdisciplinary plan of care, teach appropriate palliative interventions to family members, assess and implement a pain/symptom management program to enhance the quality of life, and provide the highest level of hospice care to our patients.
· He/she will be responsible for all appropriate documentation and reporting requirements like submitting NOE and HIS reports on a timely manner
· CM will be in charge of coordinating admission of patients
· CM will be in charge of updating patient’s individual plan of care (initial and ongoing) every IDT
· CM will obtain orders from physicians and relay necessary actions to assure coordination of services, including medication refills and new medication orders and providing other needed services like podiatrist, physical/occupational/speech therapy and wound specialist.
· CM will log and report any changes pertaining to the patient on daily basis, as well as assisting with orientation and training of new personnel and participating in IDG meetings.
· He/she will participate on rotating on-call during night, weekends and holidays.
· CM will be supervising the patient intake coordinator and QA personnel
Ø Current LVN license in California
Ø Valid driver's license, auto insurance, and reliable vehicle
Ø CPR/First Aid Certified
Ø Provide current Health/Physical Exam including TB test result
Ø Minimum of One Year experience in hospice, home health, clinic or health facility
Ø All candidates require the ability to work autonomously and in a team setting
Ø Excellent observational, clinical, written and verbal communication skills
Ø Understands Hospice philosophy and is comfortable providing specialized care to the terminally ill
Ø Good organizational skills and customer service skills
