Integrated Senior Care Home Health and Hospice is seeking a part-time/PRN LPN to work with and coordinate patient care with our existing care team! Previous Home Health/Hospice experience is appreciated but not required. You will have the opportunity to work directly with our case managing nurses and therapist to create an effective and efficient care plan for each individual patient.
We will be happy to provide ample training so that full understanding of the charting software, schedule, and care plans is achieved. We offer a competitive wage for the region, along with mileage reimbursement each pay period.
