Job Summary: We are seeking a dedicated and experienced Skilled Nurse (RN or LPN) to join our home health team. The ideal candidate will provide skilled nursing care to patients in their homes, ensuring individualized care plans are implemented and followed in accordance with physician orders and agency policies. Responsibilities: Provide skilled nursing care in accordance with the physician’s plan of care and agency protocols. Conduct comprehensive assessments of patients’ physical, emotional, and environmental needs. Monitor and document patients’ condition, response to treatment, and progress. Administer medications, treatments, wound care, and IV therapy as needed. Educate patients and caregivers on disease processes, medication administration, and health maintenance. Coordinate care with physicians, therapists, case managers, and other healthcare professionals. Maintain accurate and timely clinical documentation. Adhere to infection control and safety procedures in the home setting. Participate in case conferences and staff meetings as required. Qualifications: License: Current and unrestricted RN or LPN license in PA. Experience: Minimum of 1 year of nursing experience (home health preferred). Certifications: BLS/CPR certification required. Transportation: Reliable transportation, valid driver’s license, and proof of auto insurance. Other: Strong critical thinking, communication, and organizational skills. Ability to work independently.
