Home Health Aide

-Follow the patient’s plan of care for completion of tasks assigned to a Home Health Aide. -Help the client to maintain good personal hygiene and assist in maintaining a healthful, safe environment. -Plan and prepare nutritious meals. -Assist the client with ambulation, as approved. -Encourage the client to become as independent as possible, according to the care plan. -Observe and report any changes in the client's mental, or physical condition, or in the home situation, to the supervisor. -Perform routine housekeeping tasks, as related to a safe and comfortable environment for the client. -Comply with infection prevention and control policies and procedures. -Document visit report a in the client record. Work with personnel of other community agencies involved in the client's care. -Confer with the Supervisor about client’s status and care and participate in the planning of service and promptly report any change in client condition, to the Supervisor.
