LPN- ALF or SNF

Cypress Cove is looking for dynamic, compassionate LPN's to service our resident community! Both Skilled Nursing and Assisted Living opportunities available! Our LPN's implement resident care according to the resident’s needs and physician/nursing orders ensuring that the highest quality of nursing care is delivered. Responsibilities include: - Prioritizes, plans and provides direct resident care including the physical and psychological care of a resident group during assigned shift. Resident care may include lifting, positioning or transferring residents that require physical effort. - Participates in data collection and contributes to the care by providing data obtained during resident care. - Evaluates the response of the resident to specific nursing interventions. - Contributes to the resident discharge plan. - Provides direction for assisting personnel assigned to work with residents. - Using the nursing process in providing resident care, including making daily assessments of residents and communicates this appropriately. - Adjusts care in consultation with other healthcare disciplines as appropriate and implements plan to achieve optimal outcomes. - Performs accurate and ongoing data collection relevant to the resident’s individual needs and following appropriate protocol. - Communicate appropriate information to healthcare team members, assigned residents and families. - Supports an environment for quality care and professional practice. * Documents on residents as necessary, to include ECC. - Able to respond to emergency calls in Independent Living, including assessing, first aid or transport via EMS.
