Licensed Practical Nurse (LPN) Job Description
JOB DESCRIPTION LISCENSED PRACTICAL NURSE 1. LPN TO COLLECT DATA AND PERFORM FOCUSED NURSING ASSESSMENTS OF THE HEALTH STATUS OF AN INDIVIDUAL. 2. LVN TO PARTICIPATE IN THE PLANNING OF THE NURSING CARE NEEDS OF AN INDIVIDUAL. 3. LVN TO PARTICIPATE IN HEALTH TEACHING AND COUNSELING TO PROMOTE, ATTAIN, AND MAINTAIN THE OPTIMUM HEALTH LEVEL OF AN INDIVIDUAL. 4. LVN TO ASSIST IN THE EVALUATION OF AN INDIVIDUAL’S RESPONSE TO A NURSING INTERVENTION AND THE IDENTIFICATION OF AN INDIVIDUAL’S NEEDS. 5. LVN TO ENGAGE IN OTHER ACTS THAT REQUIRE EDUCATION AND TRAININGAS PRESCRIBED BY BOARD RULES, COMMENSURATE WITH THE NURSE’S EXPERIENCE, CONTINUING EDUCATION, AND DEMONSTRATED COMPENTENCY. 6. LVN TO PERFORM OTHER ACTIVITIES AND DUTIES AS DEEMED BY MANAGEMENT AND THE DIRECTOR OF NURSING AS DEEMED NECESSARY. 7. LVN TO OBTAIN VITAL SIGNS, WEIGHT AND WOUND CARE AS DIRECTED BY THE PATIENT PLAN OF CARE. 8. LVN TO DOCUMENT ACTIVITIES PERFORMED IN THE PATIENT HOME VISIT ON NURSE’S NOTES, COMMMUNICATION SHEETS AND ON PATIENT VISIT RECORDS. 9. LVN TO REPORT ALL ABNORMALITIES AND CHANGES IN CONDITION IN PATIENT IN A TIMELY MANNER TO OFFICE AND PHYSICIAN FOR CONTINUITY OF CARE FOR PATIENT. 10. LVN TO TURN IN ALL DOCUMENTS FOR PATIENT CARE IN A TIMELY MANNER. 11. LVN TO PROPERLY SECURE ALL SUPPLIES AND WOUND CARE FOR PATIENT AS ORDERED BEFORE PATIENT VISIT FROM OFFICE.LVN TO PROPERLY DOCUMENT ALL WOUND CARE SUPPLIES PERCURED FOR PATIENT AND WOUND CARE PERFORMED.
