Licensed Practical Nurses

CAS Home Health Care is currently seeking highly motivated Licensed Practical Nurses who are knowledgeable and eager to provide personal, home and community-based health care. CAS Home Health Care, Inc. serves Philadelphia, Montgomery, Lancaster, Delaware, and Chester Counties. CAS Home Health Care LPNs provide skilled nursing care to patients requiring daily and weekly care. All services are performed in accordance with a physician’s order. Various shifts are available. RESPONSIBILITIES: The Licensed Practical Nurse provides consumer services assigned by and according to CAS Home Health Care policies Prepares & submit clinical and progress notes accurately and efficiently Assists the registered nurse, and Physician in performing specialized services by adhering to the patient's care plan Monitor, analyze, provide assistance and direction to field staff Reports all changes in the client’s condition to the case manager in timely manner QUALIFICATIONS: Current professional license in the Commonwealth of Pennsylvania At least one-year experience in long term care or in medical/surgical hospital experience Must comply with accepted home health care standards and practices Must have good physical stamina and mental health Must present a pre-employment health clearance Must be self-directed with the ability to work with minimal supervision must have good organizational skills Must be flexible and cooperative in fulfilling role obligation Must make available the following documents: Photo ID Social Security Card LPN License Must be willing to undergo PA State Background Check process 2 Step PPD (and any follow ups 1-Step PPD's if applicable) Chest x-ray and or QuantiFERON-TB Gold is also accepted. Updated and completed physical Job Types: various shifts available Salary wage $27.50 to $29.50 /hour plus mileage depending on experience Job Type: Contract
