Job description QUALITY ASSURANCE COORDINATOR • Responsible for the implementation and monitoring of the organization’s quality assurance program. • Ensures that there are processes in place to monitor and measure all activities related to Quality Assurance / Quality Improvement • OASIS chart review and auditing • Coordinates activities related to clinical record review for data collection. • Works collaboratively with the nurses to assure documentation is complete and consistent with care and reflects legal requirements. • Provide education and feedback on nurses regarding to documentation • Monitor organization compliance with regulatory and accreditation standards. • Maintains current knowledge related to Medicare Conditions of Participation and the Department of Health (DOH) standards to ensure that the organization is compliant with state, federal and accreditation guidelines. • Acts as a resource to nurses regarding standards, policies, regulations and provides in-service training to leadership and personnel as needed in these areas. • Review documentation in WELLSKY formerly KINNSER SOFTWARE EMR and DATA SOFT LOGIC (DSL) QUALITY IMPROVEMENT SPECIALIST • Gathering data from the facility related to clinical results and more • Analyzing the different methods that care is delivered and managed • Outreach provider offices regarding reports and medical records • Analyze and prepare member and provider chase list for quality • Validate measures in charts for health plan audits • Outbound member reminder calls initiatives for quality measures • Ensure patient privacy is protected by adhering to HIPAA standards, company policies and procedures. QUALIFICATIONS: • Current license as a Licensed Vocational Nurse / LVN • Home Health experience as a Licensed Vocational Nurse / LVN required • Current BCLS certification • Current CPR card • Current TB test • Valid Driver’s license and reliable automobile • Proof of current automobile insurance (MVR checked) • Criminal background check clearance
