LVN - FIELD NURSE

Licensed Vocational Nurse, per the state requirements Good communication skills to be able to relate professionally and positively to patients, caregivers, colleagues and physicians and strong clinical documentation skills Primary function is to administer skilled nursing care, under the supervision of a registered nurse for patients in their place of residence, coordinate care with the interdisciplinary team and patient/caregivers This position will promote patients' potential during their home health episode and collaborate with physician, clinical team and patient/family members.
