Experienced home visiting LVN

Looking for a mature LVN with at least 5 years experience. Applicant will assist case manager with all aspects of geriatric care management. He/she will visit patients in home or facilities, accompany clients to appointments, review medication lists, make recommendations as needed, and file timely reports. Required: • Must have proof of current LVN license. • Must have experience working with geriatric patients. • Must have good communication skills. • Must be willing to travel around the San Fernando Valley, West Los Angeles, and surrounding areas. • Must have own car, current license, clean driving record and proof of insurance. • Must be flexible in taking occasional weekend appointments and attending weekly staff meetings. Recommended: • Experience with dementia patients. • Live in San Fernando Valley or surrounding area. • Familiarity of home health and hospice. • Good writing skills.
