Long Term Patient Care in a Patient's own home

We are looking for nurses LVN/LPN to provide home nursing care to one of our long term patients, currently receiving 24/7 from our agency. Patient is a quad needed assistance with ADLs, Transfers via Hoyer Lyft, Bowel Program, Wound Care (if needed), medication administration, Straight Cath/ICP, Reposition, accompany patient to his appointments and companion care. Shifts are 12 hours, we are looking for bothe Overnight and Day time, please note all nurses in our agency work at least 1-2 weekends per month. Shifts are 7-7. Our agency offers a personal 401k program and health insurance to all full time employees (working 36 hrs or more weekly), we offer overtime, PTO, Vacation and sick days pay.
