Personal Care Assistant (PCA) Job Description
Provides personal care to home-bound patients in accordance with Agency care plan. Provide personal care to home-bound patients in accordance with Agency care plan. PATIENT CARE A. Provides bathing (tub or shower), routine skin care, general mouth care (including dentures), grooming and shampooing hair, nail filing (no clipping or cuticle cutting), assists with dressing and toileting, and back rubs. B. Vacuums, sweeps, dusts, mops, empties trash and garbage, does laundry, cleans rooms, changes linens, defrosts refrigerator, and cleans range, irons clothes. C. Assists patient with those medications which are self-administered and prescribed by the patient's physician. Assistance may include reminding the patient to take the medication, opening the bottle of medication, providing a glass of water, and/or measuring utensils for the patient. Reports all over-the-counter medications used by the patient to the supervisor so these may be included in the patient's record. Reports all concerns regarding number or frequency in use of o-t-c medications to supervisor. D. Provides assistance to patient getting in and out of bed/wheelchair; assists with walking, encourages physical activity. E. Assists in preparation and eating of meals and cleaning of dishes and utensils; encourages proper nutrition. F. Observes and reports changes in patient's condition, applies first aid when appropriate, arranges trips to physician, picks up prescriptions, accompanies patient on medical appointments, documents patient's liquid intake/output. G. Assists with grocery shopping, bill paying, food stamp application and/or acquisition, and scheduling medical appointments. OTHER DUTIES AS ASSIGNED
