Registered Nurse Case Manager - Home Care - Fond du Lac/Dodge Counties

Patient homes include private residences, assisted living facilities, memory care units, and resident care apartment complexes. Preceptor strives to provide high quality care through our multifaceted programs including home health, hospice, palliative care, and outpatient services. We have branch offices located in Germantown, Fond du Lac, Waukesha, and Appleton. We are currently in need of a Full-Time Home Health RN Case Manager for our Fond du Lac and Dodge County territories. Full-Time, 32-40 hours per week SIGN-ON BONUS UP TO $5,000 PAID IN INSTALLMENTS MINIMAL ON CALL IS REQUIRED!  Job Requirements: Carry a current licensure in field of practice in the State of Wisconsin Excellent communication skills with patients, families, facility staff, doctors, and teammates Ability to work independently Good organization and effective time management skills Willingness to travel to meet patient needs Compliance with documentation standards and regulations Prior home health experience preferred, but does not affect eligibility to apply Additional Information: Health, dental, vision and life insurance offered Flexibility in scheduling to aide in work life balance Paid mileage Accrued PTO and sick time 6 paid holidays/year 401K with employer match We would love the opportunity to add you to our team!
