PRimary Home Health Care Inc. is a Registered Nurse owned Home Health Care Agency seeking an IV certified Registered Nurse, Case Manager, to join our team! Excellent clinical skills, and knowledge of Medicare/Medicaid regulation, and home health experience preferred. Be part of our quality team! Job Description: Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing. Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters. Communicates significant changes in client status to physician and other members of the team regarding the continuity of patient care. Collaborates patient care with auxiliary team members. Maintains client records showing systematic assessment, planning intervention and evaluation. Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in home care situations. Qualifications: Current Registered Nurse license in the state of Colorado. CPR Certified Strong verbal, written and interpersonal communication skills. Demonstrates time management, organizational skills, and ability to function with minimal direction. Valid driver’s license, car insurance and reliable transportation. Home Health experience Pay Rates are negotiable. Visits are paid based on type of visit / skill level. **Sign on Bonus will be given after hours worked are equivalent to 90 days at FT hours** Applicants must meet all federal & state mandated vaccination, testing, and screening requirements.
