Care Manager

Our office is located just a short drive north of Grand Rapids and a short drive south of Big Rapids in Howard City. We are looking for RN Care Managers to see clients in Montcalm, Newaygo, Mecosta and Kent counties. You would be working in areas surrounding your residence. We do lots of wound care and home I.V. Infusion Therapy services (IV Infusions along the Lake Shore area as well Muskegon/Oceana area). Case Managers coordinate patient care with physicians and other disciplines involved with the patients care. Variety in day-to-day care makes this position unique. Wage is not hourly, but is by the type of visit for the patient which can be explained at the interview, plus mileage. On-Call rotation is approximately every 5th week which is extra reimbursement. Also paid meetings, inservices/CEU's.
