Registered Nurse (RN) Job Description
PRINCIPAL FUNCTION Provides skilled nursing care to patients as prescribed by the Physician in accordance with the policies and procedures of Saguaro Home Health Services, Inc. Gives care in a manner, which promotes quality, continuity, and safety for the patient. DUTIES AND RESPONSIBILITIES: 1. Makes the initial patient evaluation visit, regularly re-evaluates the patient’s nursing needs, and furnishes those services requiring substantial and specialized nursing skills. 2. Implements the Plan of Care policy of Saguaro and monitors patient and home health care program compliance. 3. Implements orders for medications and treatments as prescribed by the Physician and all other aspects of the nursing care plan. 4. Will ensure that all treatment orders are documented, dated, and signed by the Physician. 5. Initiates and applies appropriate preventive, therapeutic, and rehabilitative nursing procedures in accordance with the Physician’s orders and the nursing care plan. 6. Assists the patient with activities of daily living while promoting the highest level of independent functioning attainable. 7. Instruct patient and family in self-care techniques, diet, medications, and other aspects of care outlined in the Physician’s orders and/or the nursing care plan. 8. Recognized and utilizes opportunities for health care teaching with the patient and/or family members. 9. Works with family members and other members of the health care team to meet the emotional needs of the patient and family members. 10. Evaluates and maintains a safe environment for the patient. 11. Coordinates services, informs the Physician and other personnel of changes in the patient’s condition and needs. 12. Supervises ancillary personnel: renders written instructions for the patient care within the CNA’s scope of practice; determines of extraordinary supervision of the health team is needed; assures that the patient’s rehabilitative care needs are met. 13. Completes, maintains, and submits accurate and relevant clinical notes regarding the patient’s condition, and all care and treatments are given. 14. Seeks and obtains sufficient information necessary to function effectively in the health care setting assigned. 15. Participates in patient case conferences. 16. Will submit to a performance review and at least one supervised field visit yearly. 17. Will make supervisory visits to the patient’s residence at least every 60 days when Home Health Aide Services are being furnished and at least every month when skilled nursing care is being furnished by a Licensed Practical Nurse in conjunction with physical, speech or occupational therapy. 18. Provides Home Health Aide with written instructions for home care, utilizing Saguaro Home Health Service’s approved forms. 19. Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service programs QUALIFICATIONS: 1. Current licensure as a Registered Nurse in the state of Nevada. 2. At least one year of experience working as a Registered Nurse, preferably within the year prior to the date of application for employment. 3. Proof of current CPR certification. 4. Evidence of a pre-employment physical exam or certification from a licensed physician within the past 12 months prior to hire, that the person is in good physical health, is free from active tuberculosis and any other communicable disease in a contagious stage, or a copy of an annual health assessment if employed longer than one year. 5 Record of a two-step Mantoux tuberculin skin test prior to employment and an annual one-step Mantoux tuberculin skin test thereafter. An employee with a documented history of a positive Mantoux tuberculin skin test is exempt from screening with skin tests and is followed under the TB surveillance protocol, per policy. 6. Must submit to a fingerprinting screening (criminal background search).
