RN Case Manager

Position: Full time RN Casemanager The RN Casemanager functions as an integral part of the interdisciplinary team to provide physical, emotional, and spiritual support to patients and their families. Requirements: Current and valid nursing license to practice in the state of Texas Med/Surg and/or hospice Experience Reliable personal transportation Current and valid state driver license and current auto liability insurance Duties and Responsibilities: Ability to implement patient care as directed by the plan of care Attends team meetings and in-services as required Sound knowledge of pain and symptom management Ability to exercise good judgment Excellent communication skills Schedule: 8am-5pm M-F Benefits-eligible position: Health, Dental, Vision, Life, retirement savings plan, generous paid time off Job Type: Full-time
