Registered Nurse (RN) – Home Health

Luthas Health Care is hiring compassionate and skilled Registered Nurses (RNs) to provide home health care to clients across Suffolk, Plymouth, Bristol, Norfolk, Middlesex, Worcester, Essex, and Barnstable counties. You'll work closely with patients, caregivers, and interdisciplinary teams to deliver high-quality care in the comfort of clients' homes. Responsibilities -Conduct initial and follow-up in-home nursing assessments -Develop and implement individualized care plans -Administer medications, injections, wound care, and treatments -Monitor vital signs and document clinical updates -Educate patients and families on disease management and safety -Coordinate with physicians, therapists, and aides -Supervise home health aides per care plan -Support clients with DME needs, developmental disabilities, or memory care -Maintain timely, accurate EMR documentation Qualifications -Active RN license in Massachusetts -At least 1 year of clinical nursing experience (home health preferred) -Ability to work independently and travel to patient homes -Strong documentation, assessment, and critical thinking skills -Experience with memory care, chronic disease, or developmental disabilities -Proficiency with electronic documentation (EMR/mobile tools) -Valid driver’s license and reliable transportation
