Registered Nurse (RN) Job Description
Reviews medical history, radiology, lab results, to determine medical status and appropriate courses of treatment intervention and communicates these findings to the physician Coordinates the patient’s plan of care and communicates with physician, interdisciplinary team and/or Clinical Supervisor regarding patient’s pain Assesses and documents patient’s response to interventions and determines if interventions need to be changed or care discontinued Implements a plan of care through nursing interventions and skills to enhance patients and families progress towards goals and accurately documents all aspects of care on the appropriate tools as required by the department in a timely manner, evaluating the efficiency of the plan, and when changes to plan need to be made, initiating physician contact and measures to meet desired outcomes Provides on call coverage, weekend, and holiday coverage as assigned by the Clinical Supervisor Supervise and plan Home Health Aide activities appropriate to individual patient care Utilize excellent verbal & written communication Admit new patients according to department procedures Organize and implement individual patient care based on ongoing interpretation of physiological, psychological, sociological deficits affecting the state of health Case management of patient care, and admissions Coordinate and educate patient and family as needed regarding care Evaluate patient care and progress Identify areas of support for family and patient needs Attend weekly case conference and monthly case Manager meetings Coordinates case management activities within an Interdisciplinary health care team to insure appropriate delivery of services to patient Requirements A minimum of one year experience as a Registered Nurse (RN) is required Graduate of an accredited nursing nursing program Current Basic Life Saving (BLS) certification through the American Heart Association (AHA) within 30 days of hire Two (2) years recent experience in a medical/surgical general care setting, or recent experience in home health or hospice skilled nursing practice Strong computer keyboarding skills and experience working with an Electronic Medical Record (EMR) system Home Health or Home Care experience
