Our 44 bed skilled nursing facility (SNF) specializes in short-term rehabilitation and 24-hour nursing care to help our patients recover from short-term illnesses, injuries, and post-surgical care in order to return to their home. Our in-house team of therapists and nurses create individualized care plans, focusing on the goals and needs of each patient. Our skilled nursing team has experience with a wide variety of diagnoses and delivers wound care, IV therapy, medication management, and post-surgical care. An on-site physician and an ARNP direct the convenient care of each patient and our social services team coordinates discharge planning with the resident, family, and other providers.
