I have been at the nursing home for approximately 5 years now. They recently changed administrator's and her name is Stephanie and also the new DON named Joan which are doing an excellent job running the place. What I like about them is they have an open door policy for the residents and will always be there to listen to you and if you have problem they will fix it. All the nurse's are very professional and do there job's. All the GNAs are very prompt at answering the call lights and helping with whatever you need. I have only had a few issues in all the time I've been here and Stephanie or Joan would help me resolve the issues. The nursing home and rooms are very clean. If you need a nursing home and rehabilitation center for your love one to stay long term or rehab then I would place them here because I know that they will be in good hands.All the staff are very polite to everyone and they all are my family. I have to give a big shout out to the administrator Stephanie and the DON Joan which are doing and excellent job running the nursing home. I have to mention Tanchell who is the activity director and does a fantastic job. I want to mention Tracy who runs the rehab and her staff who do rehabilitation with you and get you to the point you can go home. I also want to mention the smoke aide's Beverly and Kenya who do a great job. In closing I would choose this nursing for my love one if they needed to be here. In my honest opinion I would give it 5 stars. I also want to mention Debbie who is the admissions director and does a great job at what she does kudos to her. Thanks

I Karate Davis have been to Northwest Health & Rehab on several occasions and would recommend this place for any one who needs rehab. The staff here are excellent and give good care.

My brother was in this facility for three months with end stage dementia. While the building is old and not fancy, I found it to be clean, beginning with the parking lot. The staff were kind and compassionate. The people at the front desk were usually very cheerful and gave a warm welcome upon entering. The gentleman who worked there was especially kind! Whenever I visited my brother, he was clean and it was apparent they were doing everything possible to keep him safe. I never heard any of the staff complain and usually they responded to my requests. Working in a care facility such as this is not easy. People are quick to find fault. I have chosen to dwell on the good. It can't be easy watching people you have cared for die. As my brother was dying, Theresa, the Administrator came and asked me what I needed. "Anything at all she said. Then the dietician came in with a cart full of snacks in case I wanted anything. She said, "The people here are my family!" On the whole, my experience with Northwest was a positive one.

If I could give this place a 0 star rating I would. Nurse are rude and GNA's are too overwork to care about the residents. My father in law came to this facility around March 1st and not even a week later they claim he had an unwitnessed fall. He was sent to Sinai Hospital where he needed to be intubated and is currently in the ICU. This facility is hiding the truth about what really happened because why would you leave someone who has dementia unattended and they knew my father in law was demented. Also when he was there they never changed his clothes when we went to visit he the second day he was there he still had on the same clothes and lied about not having extra clothes for him to wear. Moral of the story do not send your family members here because they will end up in the ICU or worst... Dead!

Not a good place to place your family or work the whole HR department need to be fired and replaced! Just sad how people wake up everyday and be satisfied with themselves!The state need to shut them down and restart NorthWest Rehab over again for the sake of the name and 40 years they put in that African definitely don’t have a clue Sara think she doing something by being mean to her staff wow and Rondetta do as she told don’t investigate nothing! I will keep putting reviews out til justice is served!
