Today was the first day that my father was released from the hospital and admitted to this skilled nursing facility. I called to get an update and was hung up on. Upon calling back to get an update after a few minutes I got hung up on again (someone or persons picked up and hung up, at least twice). Drove 35 minutes to the facility to address issue with the supervisor. The response from the facility supervisor was that the issue would be addressed and that there was a fire which may have caused the transfer to another nursing station. This is documention for the reviewer.

Andy Legg was in this facility for over a week during which he never saw a doctor. The best thing I can say was it was clean but communication was sorely lacking. They failed to send his prescriptions to VA even though I wrote the phone number, his SS # and DOB. Despite several calls from the VA and myself they were not sent. After he was discharged on Wednesday he was supposed to have dialysis on Friday. No arrangements were made resulting in a long wait at OSU ER for a bed and dialysis. Many calls were ignored from not only me but also the VA. On Friday afternoon after the squad took him to ER the Director finally called and assured me if Andy returned he would get excellent treatment. I was not convinced. Rehab was short and only once a day. They have some positive amenities, courtyard, an activity room and a dining area where you could order a meal. Andy flatly refuses to return there.

I’ve had the opportunity to tour this skilled nursing facility and visit some friends here multiple times, and each visit has left a consistently positive impression. The staff are professional, courteous, and genuinely attentive to residents’ needs. The environment is clean, well-organized, and welcoming, with a clear emphasis on both safety and comfort. I’ve observed strong communication among team members and a respectful, caring approach to resident interactions. It’s evident that quality care is a top priority here. Based on my repeated visits, I would feel confident recommending this facility to families seeking skilled nursing care. Thank you guys for all you do!

I have to say this was one of the worst experience I’ve had in my life. My uncle came there after being in the hospital being told he was getting ready to come home not knowing you guys weren’t paying attention to what was going on in the facility. my uncle fell still don’t have any answers to what happened Days. My uncle passed away from my father that was happening in your facility. This is not OK and unacceptable on all levels. And it’s sad this is ridiculous.

My mother was a short-term rehab patient at this facility from November through late March, and our experience was nothing short of exceptional. As someone who has worked in long-term care for over 20 years, I understand the daily challenges the profession faces—especially in a post-pandemic world. But nothing truly prepares you for being on the other side of the equation—when the person needing care is your own mother. From the moment she was admitted, the leadership team provided immediate reassurance and compassion. The Administrator made a point to say, “We’re going to take good care of her,” and that promise was kept every step of the way. The personal touches—from small comforts to consistent communication—helped turn a stressful time into something deeply meaningful for our family. The Director of Rehabilitation and her team skillfully managed my mom’s care with attentiveness and professionalism that led to incredible therapeutic outcomes. The nursing and therapy teams were attentive and responsive, recognizing critical changes in condition and acting quickly—actions that quite literally saved my mother’s life. Throughout her 140-day stay, we saw consistent dedication, kindness, and a level of service that went well beyond what’s expected. From the clinical team to support staff, everyone treated my mother with the dignity and respect every family hopes for in a time of need. This facility not only helped my mom recover and return home—it gave our family peace of mind and restored our faith in what great post-acute care can look like. We are forever grateful.
