Last week, I was sent to IL from WI to look at facilities for my mom. I arrived with my two little kiddos and was immediately greeted by the receptionist with a smile. The staff were all extremely pleasant and the facility was clean and well kept. My story is a little different but so important and shows the character of the staff here.
My mom who lives in WI was told by her SW at the hospital that there were no places willing to accept her in WI, we were given two places in IL to tour, this was one of them. I went to look here and thank gosh I did! I met Josh, who was beyond kind and knowledgeable, not only that, he reviewed my mom's records and said he might know of a place closer in WI that could help us. My mom was in the hospital for 4 months and was told IL was her only option and this kind soul, helped us in 1 hour find an amazing place in WI to accept my mom. This demonstrated how much the facility cares and values a human life and truly wants to help people.
I will forever be grateful for the knowledge and compassion that was shown to my family in this difficult time, and the willingness of Josh to go above and beyond to help a complete stranger so that her loved one could be closer to her family. 10 out of 10 recommend!

Zahav takes excellent care of the residents. The nursing staff are top notch. Physical and occupational therapists increased my stamina and strength to where I now work out daily. I call myself a therapy junkie. The therapists work with the residents on the skills they need. That’s what they did for me. In Activities we celebrate all the holidays even the more uncommon ones like Mimosa Day and Ice Cream Sundae Day. Access to the heads of all the departments is available for all residents and that is highly unusual. Having been in several rehab and nursing settings, Zahav is the best.

I am a friend of Jerry on room 125, this is my first time visiting and am impressed with the cleanliness , no smell , that is a big thing. Very friendly environment , from reception , speech therapist, very attentive and impressive with details, he is clean and well taken cared of, he told he loves it here. The Assistant Director was extremely friendly and caring . I highly recommend this place. Very impressive. I have a massive email with my friends and I will recommend this place. Keep up the great job

Man… where do I even start? I came to Rivaya (it was called Landmark back then) back in 2022. I was in a coma, on a vent, couldn’t move, nothin'. I was in bed for over a year. Thought I’d never leave. But the people there… they saved my life.
Ownership changed a couple times... was Zahav for a bit, now it’s Rivaya. Gotta say, Rivaya’s been great. They really seem to care about doin things right.
The respiratory team was just top notch. I mean, really knew what they was doing. The CNAs... God bless 'em... always treated me good. Nurses too, always there when I needed something. Everybody just cared.
Therapy there was amazing. I had a G-tube for a long time.... couldn’t swallow, couldn’t eat right. They worked with me every day, helped me learn how to swallow again, get stronger. I'm even walking short distances now. I mean, I’m still in a mobility scooter most of the time, but I can get around some on my own. Feels like a walking miracle, if you ask me.
I was even the president of the resident council for a bit, which meant a lot to me.
Activities were amazing too... always somethin' going on. I even got a certificate once for 'Resident of the Month,' which was awesome. Made me feel real proud. Gave me a purpose while I was healing up.
Food was actually real good too. I looked forward to it every day, no joke.
Now I’m writing this from my own apartment. Been out 3 weeks. I'm out, movin' around, doing my own thing. Got my life back. I can’t say enough how grateful I am.
I still come back almost every day to read the Bible with the residents. It’s just an 11-minute ride on my scooter! Feels good to give back.

The receptionist who answers the phone is rude, sounds like she cant be bothered. No one at the nurses station will answer the phone when I call to check on mom. They are supposed to give her restorative therapy but they don't. At all. My mom is wasting away in bed because they won't do any restorative therapy, even though I was promised they would do all the can to get her out of bed. This place sucks don't let your loved ones stay here.
