Last time I was there visiting a resident 3 weeks ago, one of the staff was mocking one of the clients who yells out repetitively. very unprofessional. It smells to high heaven and there's always full urnals by his bedside. We do his laundry for him since his items never come back and is replaced by worn, torn, and stained clothing. What happened to the nice new track suits and sweats I just bought him? They were marked. Now they're trying to change his insurance so they can keep him? He's only there for rehab and he's last stay there landed him in the ICU on a ventilator! He's there for a bed sore on his foot! We're trying to get him home but they won't return our calls and reported us to APS, trying to keep him in there. If the administrator cared about the patients as much as she cares about her little french bulldog she pushes around in a cart, it would be a different place.

Worst place ever, my father in law has been there for months, now that they sucked up all his va time, they want to change his insurance to take more money from this veteran. I get notified only a day before he's supposed to be out at 3:30 in the afternoon and everything is closed to actually get him home. They then deemed it unsafe for him to leave, for two weeks I've been calling this place to set up a safe discharge home for him with no response from anyone. Only to aps get involved and for him to be totally confused on why they won't send him home. NEVER WILL HE GO HERE AGAIN. Oh yeah that's once they actually let him go home!

I am a resident of a nearby apartment complex. During a recent nighttime thunderstorm, a man discovered an elderly woman from your facility alone in her wheelchair, sitting in the pouring rain in the parking lot—approximately 500 feet from the building. We were able to help her into his car and brought her wheelchair back with us. When we returned her to the facility, I voiced my concern to a staff member, asking how such a situation could occur. Rather than expressing concern or appreciation, she responded with a dismissive comment, stating that staff do not track residents and are not responsible for ensuring they are in their rooms at night. She later added that the woman “shouldn’t wheel herself out in the rain.” This response was deeply troubling. A vulnerable, paralyzed elderly woman should never be left alone outside in dangerous conditions—especially not at 9:30 p.m. during a thunderstorm. The lack of compassion, accountability, and basic gratitude from the staff member was shocking. If my loved one were a resident here, I would be extremely distressed by this incident. I urge you to take this matter seriously and implement better safeguards to protect the residents in your care

My mother has been a patient here for the past week. I am very thankful this is the location where she was she was placed. The facility is old. But the staff here is fantastic. Each and every staff member is so nice. They truly exhibit a caring and love for the patients. The staff is very attentive to the needs of the patients. They show great respect for the dignity of their patients. I have been truly pleased with my mother's stay here. She also comments always on how nice everyone is Special call out to Angela, Bernard, Sandra, and Lucy whose care, kindness and compassion go far beyond the norm.

I've had both parents in this facility, my mother was in in 2012 and we had a great experience at that time so when my dad needed a place to recover and get rehab we thought of this facility. It has not disappointed. The employees are wonderful and very caring and treat him like family. They have a lot of different activities and most Fridays they have outings to different places such as Walmart, bowling and we just went to Uncle John's Cider Mill. We were just at their annual Halloween party and it was great fun. They had food and a musician and even brought in animals for everyone to pet and enjoy such as cats, dogs, bunnies and horses. It was a lot of fun for the residents and their families.
