This facility needs to be investigated and shut down completely. I have never witnessed so much unprofessionalism in one facility. This facility had two jobs to do for my mother and failed terribly. My mother was there all of 3 days and I saw enough. Day 1: she arrived to the reserve from AMG in Mt. Pleasant, wonderful place and staff! I get to the reserve check in upfront and proceed to my mother’s room. Upon entering the room it smelled like feces.. I checked her bathroom to see if soiled briefs were left behind. That was not the case but her bathroom was filthy. A used urinal was beside the toilet along with a crusted washcloth. Mind you a male was not present in the room for it to be a urinal in the bathroom.. My mother’s roommate pleased with me to go find someone to change her and give her some water.. she stated she was waiting over 3 hours. I went to the nurses station where everyone was congregating talking about their personal life to be ignored until I finally said hey a resident needs to be changed and she needs some water. One young lady said oh yeah I forgot to tell you your resident needed to be changed and she asked for water. That was strike one Day 2: I walked in my mothers room and her bed was lifted way to high for any facility guidelines and her leg was hanging off the side of the bed.. my mother is not mobile and is a fall risk! I lowered her bed and adjusted her in it to pull back the sheets to see she was soiled. I went to the nurses station to get assistance in toileting my mother.. I assisted the young lady and proceeded into the bathroom to wash my hands to see the same urinal and crusted washcloth along with the trash over flowing. That was strike 2 Day3: My mother stated she went out to be Dialyzed and they turned her around due to lack of paperwork from the facility. At this time my mother is swelling holding fluid. I went to the nurses station once again to be told they don’t know when she will be dialyzed. I stated she needs to be sent out to the hospital because her missing treatments are detrimental. I stayed until 11 pm and things went downhill with the oncoming shift. My mother stated she was dizzy. I did not want to leave her unattended so I pushed her callbell.. 10 minutes went by and I went to the nurses station where everyone was laughing and talking while the call bells where flashing going off. I said my mother vitals need to be checked because she has a history of bottoming out or blood sugar through the roof.. it took 15 minutes for the medication tech to come. Her blood sugar was over 300! She needed insulin with took another 10 minutes to get. I lost it on all the staff members letting them know they need to take their jobs more serious and administer proper care to these people.. My spirit was so bothered.. Day 4: I called looking for my mom and nobody knew where she was. I called the clinic to be told no they discharged her and her seat time was no longer available. I called the facility again and nothing.. I called the hospitals and finally they told me she’s been there since 8:30am! It was 11am when I found her.. I went to the hospital to make sure she was okay! I then went to The Reserve to get my mother belongings and to let them know she will not be returning. I told management the facility is not fit for anyone. I wouldn’t let my DEAD dog come here and she’s deceased. Please be an advocate for your loved ones and get them placed somewhere else. This place just wants the funding! I will be looking into taking legal action due to poor care and my mother having to be admitted to the hospital due to the facility not providing paperwork for her to be dialyzed so her body retained fluid and other things

Wow what can I say. I was transferred to the Reserve from Summerville medical center so I could get Physical Rehabilitation since I had just had me leg amputated. Biggest mistake I could have made was to not say NO. They got me into the room and were supposed to do a workup on me, check for cuts and bruises and things. But it never happened. One day a nurse comes in and asks me what arm did the do a TB test on. This was confusing since they never did that test. Now remember this is supposed to be a Healthcare facility which should be as clean as possible. It wasn't.i was there for 15 useless days and my room was never swept or mopped. It was disgusting.my food when I got any was cold and the iced tea frozen. There was a flue outbreak there and I never got down to the rehab room for what I needed. There is so much more to say that's negative about this establishment but I just don't have the energy to write it. There were a couple great nurses and the PT and OT people were fantastic. This was the homes savings grace. But I will die before going back to the Reserve. So if you have a loved one who needs this type of place DO NOT PLACE them here

So far so good! My mom just started going here and the staff and nurses have been so helpful in helping me in my mom in her new journey. No place is better than home but they try to accommodate and make her as comfortable as possible. Special shout out to Stephanie the admissions lady that greeted me and walked me through the process from day one and always checks on my mom to try to make sure she is transitioning well. Nurse Brandy who truly knows her stuff and is knowledgeable about dementia patients. To everyone, everyone has been so great and so kind with my mom and I. I cannot name them all. I pray it continues because I truly truly needed the help in dealing with her.

Despicable! My mom got a little bit better, however, they never told me what a fall risk she is and that she couldn’t be alone. Because of this, she suffered an injury to her head, no medical treatment occurred. Moreover, I CANNOT BELIEVE they let a still physically disabled woman that they (and other doctors) also deemed cognitively impaired discharge herself and leave in a cab. Staff was notified that family was coming to get her. They watched her leave without all of her belongings and her necessary back brace that they gave her and pestered her to wear. And now, the items she forgot to take with her are mysteriously missing. Two hundred dollars worth of clothes, phone and charger, and her whole purse. Icing on the cake is that she didn’t have sheets for at least two days or help changing her clothes for her entire stay. She left in what she arrived in. They don’t care or take note of anything…you are just another number and they want to “turn tables.” Despicable!

NO STARS if it was up to me 😡I'm absolutely heartbroken from this place. What started out good went down hill fast. When my brother was sick and had to be transported to Trinity Medical for treatment according who is on duty I would be notified. But the last few times he has went no one communicated to let me know when he had departure/arrived back to the facility. I live in another state so on one visit when I traveled to see him I went straight to the hospital to find out he was sent back two days ago. I would call the facility and no one would answer the phone. This has happened a few times on non communication. This is the kicker. My brother transitioned July 26th 2025 at Trinity Medical Hospital. One of the hospital staff called me 6:24 a.m that morning to tell me he had passed away. Today is August 13th, 2025 and NO ONE from the facility has yet to call me and tell me that my brother passed away nor has called to give there condolences 🥺😭. I highly recommend you NOT take your love ones here !!!!!!!!!!!!
