So far, so great.
My mom was diagnosed with dementia a while back, and recently, my stepdad could no longer care for her at home. It fell onto me, her eldest son, to take on the role of guardianship.
I contacted over 20 facilities in the NEPA area to see where my mom could fit in. Of those, a handful responded, and after careful consideration, I narrowed it down to The Pavilion at St. Luke's.
Dana in admissions was super helpful, and answered all of my questions. We took our time, and over a couple weeks to planned my mom's admission, which took place just days ago.
Admission day was emotionally stressful for us as a family, but all the staff there showed care and understanding of the situation. My mom was made welcome and comfortable in a shared room.
FF a few days, and Mom had a moment where the new staff wasn't confident how to handle. They called me, late on a Sunday to see if we could help her together. Lee Anne, the supervisor on duty, took at least a half hour on the phone with me to see if we could help my mom together. I appreciated that very much.
I hope the kind of care they have exhibited so far, will continue for mom.

Poor communication. The administrator does not follow through when she suggests orders for the residents. The nurses are of no help when told about resident neglect . grievances are lost . snacks are labeled without care of likes or dislikes. Most brakes are defective on the wheelchairs resulting in falls.Making fire drills a concern. Residents can be left dirty for hours. One person assist for residents needing hoyer lift even though the administratior dictates 3 man help. Residents are changed in wheelchairs when a Hoyer is needed to be changed in bed, resulting falls. grievances are lost...care plan meetings carry on as the staff pleases.

If you care about your loved ones, do NOT send them to St. Luke’s Manor in Hazleton, PA—or its affiliated locations, Forest Hills and St. Luke’s Pavilion. These facilities are riddled with neglect, mismanagement, and outright mistreatment of residents.
🔴 Dangerous & Unsanitary Conditions
* The showers are filthy, mold-ridden, and broken, with gaping holes that allow water to leak into linen storage areas, creating an unsanitary, unsafe environment.
* Hot water is unreliable, meaning residents are often denied proper hygiene and given rushed bed baths instead.
* A stench of mold lingers in the air—this place is not clean or safe.
🆘 Critical Needs Are Ignored
* If a resident has special nutritional needs, this facility REFUSES to accommodate them—no matter how medically necessary. Despite thousands of dollars being spent per patient daily, basic care is withheld unless families pay extra out of pocket.
* Even essential medical and hygiene items are denied, leaving residents vulnerable and suffering.
💀 Understaffed & Abusive Treatment
* Residents are roughly handled, with some left with raw, damaged skin due to negligent care.
* The facility is severely understaffed, meaning residents often sit in soiled clothing, miss meals, or receive improper medical attention.
* The higher-ups—especially those running the show—are verbally abusive and threatening. They speak down to the elderly, knowing these vulnerable individuals have no way to fight back.
⚠️ A Facility Built on Greed, Not Care
* The company behind these facilities has a history of violations, including fines, bankruptcy, and infectious disease outbreaks.
* Patients are often kept longer than needed just to fill beds and milk more money. This is NOT about care—it’s about profit.

This is not just a single bad experience—it is a pattern of systemic neglect and abuse. If you or a loved one are considering a short-term stay, think twice.
🚨 Report them. Speak out. Demand better. Our elderly deserve dignity, not to be trapped in a facility that treats them as disposable. Do your research before making a devastating mistake.

The staff at the Pavilion is TOP NOTCH. Kind, caring and compassionate to say the very least. Every time I am in the facility to visit my family member, I am greeted with a smile. My uncle is always clean, well kempt and well fed! He's treated like gold and safe. This is the skilled nursing home to pick for your loved one!

I have found that the pavilion staff offers the most professional, respectful, caring and kind care that my family has found anywhere. My aunt is always clean and smiling. She is encouraged to participate in daily activities, her needs are met promptly in a kindly manner. The staff is never too busy to answer any questions that we may have,and the facility in general is always very clean . We are deeply grateful for the staff at St. Luke’s Pavilion!
