We were pleased with the appearance and cleanliness of the assisted living rooms and dining room; however, after our Mother lived there a few months things became an almost daily battle. The housekeeping and laundry services were non-existent; there was no consistent person to communicate with; the staff was often absent from their posts/work and had no intention of caring for Mom as she required. After our mother was moved to skilled nursing, the care she received there was better, due to the fact that we got Hospice involved. The rooms on that side of the building are very rundown and dated. At least they do have some qualified nursing staff for that area, which is the facility's only saving grace.

I've only been twice to visit a patient. I used to take care of him at his home. His daughter said, he misses me. So I went to visit & the common areas are nice & clean. But his room was dirty. I couldn't take it & i cleaned the kitchen, bathroom & made the bed. That took almost 2hours. I told the daughter about it & she called & complained.

Wendy Rimel went above and beyond to keep our family member taken care of during her care there. Wendy kept constant communication with the family and gave us peace that she would love and care for our family all the way until the end. It made an impact on us during a hard time. This facility is truly blessed to have her!

Every one of the poor reviews is unfortunately accurate and exactly the level of care you can expect to receive here. We have filed complaints with their corporate office as well as CMS. They will over-medicate your loved one in order to keep them compliant and quiet. Please review your options and believe what you read in their reviews. My grandmother was neglected and over-medicated here.

If you want your family member to be neglected and sent back to the hospital in worse condition than they were before arriving at Oak Tree…go ahead and send them there. Negative 100 stars. HORRIBLE experience. The administrator has an attitude problem, she rushed us through the initial walk-thru and was not helpful or friendly at all. She gave us a whole 10 minutes of her time. We felt backed into a corner because our mother needed an inpatient PT facility and they were the “best” of our 3 options. Literally only one CNA on their staff that was compassionate and competent. Her name was Katherine. They allowed a medication technician attempt to assist my mother to the bathroom who has a broken patella and was non-weight bearing and high risk for falls…she let her fall. In fact she said “just fall to the ground” when my mother said she was losing her balance. Her leg was broken again only worse this time. EMT was called because blood was gushing out of her knee and she was in severe pain…now she’s back in the hospital after a 3 week stint here prior. She’s had a 3rd surgery and will being have a 4th because of their negligence. She will never walk again according to the surgeon. Good luck!!
