Of course no place is perfect, but I will say that this place has a heart of compassion. I've watched the the employees go above and beyond for the residents and family members. I just want to say thank you. And I would like to send a special shout out to the new administrator Kristian Carr because it starts with the head of the leadership team. I appreciate all the effort. I just want to give thanks to the new Administrator, Kristian. She is always pleasant and welcoming family members to Spring Gate. She is often mistaken as an employee but before they ask who are you she replies “I am the new Administrator and if you have any questions, I can be reached at this number. Her reassurance, leadership, and changes has given me a piece of mind in knowing that my Loved One will be taken care of. Thank you all for your service and dedicated staff.

Spring Gate Rehabilitation Center was not my first choice for my husband, but it was one of the few options we had. I trusted that he would be cared for with dignity and compassion, but sadly, that was not our experience. I was there faithfully everyday. His belongings were stolen two weeks prior to his passing. The facility was suppose to reimburse me—it has been nearly four months no results. There were a few, and I do mean a few, good and caring people who worked there, and I am truly grateful for them. But my heart breaks for the other patients still their. Thanks for responding. However, they have done a report, I sat and talked with someone at the facility. Also, communicated through email with someone in social services. They came up with a resolution but I have not received anything and it's been 4 months now.

LOVE LOVE LOVE LOVE! My family member is getting loved on when I cannot. We had an amazing experience meeting and speaking with the staff at Spring Gate! Love the way the receptionist is so friendly and kind. I am very impressed! I love that she is close to home and getting the care that she needs. Thank you Spring Gate!

Extremely angered and disappointed with the actions of Lavita Worthy at Spring Gate Rehabilitation and Health Care Center. On August 5, 2025 my father and our family were purposely misinformed/mislead in regard to the transportation of our father moving to an another facility. Ms. Worthy refused to acknowledge our family’s and my father’s repeated requests in attempts to ascertain information in regard to the transportation due to the fact the transportation was over 5 hours late. Mrs. Worthy attempted to suggest that we (the family) transport our father ourselves. Mrs. Worthy was advised that we did not feel comfortable in doing such due to my father’s declining health and instability however, we would be driving from Louisiana (a six hour drive) and would be picking his personal belongings up and bringing same to the new facility in hopes of making the transport easier. Upon our arrival six hours later my father had yet to be transported. During those six hours my father, our family, and the new facility attempted to contact Mrs. Worthy via public service numerous times and she would not acknowledge our repeated request for information. Mrs. Worthy was present at the facility and did receive the messages for we were told by numerous employees she was aware of our repeated calls. I feel Mrs. Worthy’s intentions from the get go was for us to transport our father upon our arrival at Spring Gate even though we expressed our concern with doing such. Well Mrs. Worthy basically won for upon our arrival our father was so distraught we couldn’t with a good heart leave him there not knowing what would be now that is was 6:00pm and coincidentally Mrs. Worthy was now off. (how convenient) I am very disappointed in the unprofessional actions of Mrs. Worthy. I do hope that if she is blessed enough to still have her loved ones in her life and one day finds herself in the same situation as our family was she is not met with the same resistance and total lack of empathy as we were.
