Unfortunately my wife was there many times over the last year. The facility is absolutely spotless. The staff is very friendly, helpful and professional! The food needs a little bit more attention but not much. My wife is a very picky eater but she usually ate most of her meals. They bent over backwards to make sure she was served something that she liked.
The administration is outstanding starting at the top!!! Nursing, Physical Therapy, Occupational Therapy I don’t think could get any better! Social Services staff did excellent work and were fun to work with but still professional. Dietary was knowledgeable and kept me informed on my wife’s progress.

Mentor ridge is a beautiful center. My grandmother is currently there. All the staff is absolutely wonderful and friendly . Very helpful with any and all of my questions. My grandmother is getting wonderful care. Also The center is also very clean. Keep up the great work mentor ridge!

My dad was recently here after he had several falls at home. The staff was so kind to him and made him feel at ease. The place was clean and the food was pretty good for a facility. My dad formed relationships with the staff and was sad when he went home and still talks about some of the staff. If we ever need a rehab again, we will be coming back to mentor ridge. Therapy was fantastic as well.

If I could give this facility 0 stars I would. They almost killed my mother until we insisted she be taken to the hospital. She was found to have a huge infection originating from laying in her own feces in her bed, we don't know for how long, my sister found her in the morning. When asked to get someone to clean her up she was told they had to deliver breakfasts first.
They lied to us from day one. She received very minimal mouth care and physical therapy. When I went to the nurses station to ask what she did each day no one ever knew, "they were just coming on shift". They would plop her at the nurses station in a wheelchair and ignore her for hours.
After she was taken to the hospital they had the audacity to hold a meeting telling us that if and when she returns to the facility they will put other protocols in place for her. They knew at that point they were not taking her back, nor would we have taken her back there.
No calls back, no communication, unqualified so-called medical professionals, therapy that I could have done without a degree, and major patronizing or, if you will, lying, to a very involved family.
My goal will be to share this information with anyone so their loved one doesnt go thru what our family went thru.
They should have been honest from the beginning. They didn't have the knowledge, expertise, or even compassion to take on such a complex patient, but they got the Medicare money and I guess that was THEIR goal from the start.

DO NOT send anyone you love here. If I could give negative stars my family would, but we can't so here's our one star review:
This facility may look perfect from the outside—clean, modern, and spacious—but the reality is horrific. Our mother was admitted for a pleural effusion while also battling stage IV cancer, which involved extremely sensitive medical needs. In just one week, she endured shocking neglect and abuse:
-Staff were rough and dismissive while cleaning her, even after she repeatedly explained that the area was painful and sensitive.
-She was left sitting in a wheelchair for over 90 minutes because a staff member "had to deliver food." My uncle had to physically come from home to help her back into bed.
-The most egregious behavior came from a blonde, 40-ish dietary aide - we are unsure of her title, but she was repeatedly dropping off food (her name tag was always missing and/or turned around). She was openly cruel and should never be in a patient-facing role. Because my mom was not eating, my uncle requested a vanilla Boost (high calorie nutrient shake) to be brought for meals. I personally was told by this "aide" that "we have to by law bring food" and "a Boost is not a meal, what are you talking about **aggressive sigh and shrug**. I personally asked said "aide" what she thinks we should do, because my mom isn't eating any food. She proceeds to shrug and sigh again and states "well I don't know I can ask the nurse", slamming the door on her way out. Approximately 20 minutes later she proceeds to slam a warm, chocolate boost on my moms side table and storms out of the room, slamming the door shut again.
-Despite being stage IV and in severe pain, it took hours for her to receive prescribed pain medication. *at this time our family is unsure if my mom was even given correct pain medications (we believe she was only given Tylenol and not her prescribed Oxycontin).
-No staff regularly checked on her—even though she was on oxygen and admitted for fluid on the lungs. ********When she first arrived, she was supposed to be on oxygen immediately, yet it took a full day before staff brought a tank (my sister had to place the cannula on my mom and she arrived the day after my mom was placed here). No family members (despite us being there for hours at a time, ever saw her vitals being checked or anyone besides janitorial staff come in).
-She eventually stopped eating and drinking altogether because going to the bathroom required waiting hours for someone to assist her, during which she felt unsafe and threatened (especially by night shift staff).
-My sister and I attempted to contact multiple “higher-ups” at the facility. Not one returned our calls. They could not (or would not) tell us who was working any given shift, refused to provide medical records, and failed to answer even the most basic questions. To this day, we do not know if a licensed medical provider ever actually examined or checked on her.
As a direct result of this facility’s negligence, our mother is now in the ICU, fighting for her life. *Our family needs to add that 8/14 both my sister and myself received a call from "Ashely Johnson" that she called 911 for our mom due to her being unable to breathe. However our mom reports that she waited over an hour for someone to come to her room after numerous call button activations. Her oxygen level was at a 70% when EMS brought her into the ER.
This place is shady, dangerous, and cruel. It must be investigated by the Ohio Medicaid /Medicare board for abuse, neglect, and fraudulent claims of treatment.
Myself, as well as my family members all witnessed this first hand; along with my mom's report and including numerous late night phone calls about how terrified she was and how "nobody is coming to help me". My uncle responded numerous times late at night to assist her because no one else would. We understand the facility was under 24/7 surveillance, so if there are any questions about the above stated incidents.. go on ahead and check your footage.
