Absolute nightmare. The legal, ethical and inhumane practices at this facility are atrocious. My mother fell and broke her arm and no one assessed her, notified us or reported it until the family complained. My mother tripped on a loop of extension cord left on the floor under the mini fridge by maintenance to which the administrator vehemently denied yet she wasn’t even there at the time of occurrence. Our family started asking about policy’s for the facility such as reporting falls and other facility “rules” we were denied the information from the Administrator. We also requested to review my mother’s medical records with 24 hours (per state guidelines) advance notice and were again denied. The facilities solution to the illegal and unethical treatment was to issue her a 30 day notice to leave based on her breaking smoking policy. The day of my mother’s surgery to fix her broken arm, the administrator ordered a urine drug screen searching for an alternate reason for the fall and initially notified my sister and I that her room smelled like meth and someone in the background said THC. She advised my mother was positive for cocaine. When I asked if there was a confirmation test done she said no. I requested a supervised urine drug screen test done at an outside facility for confirmation and yet again, we were denied. The administrator is quite the prevaricator and the social worker is extremely unprofessional and condescending. Under duress of being threaten to be “put out on the street”, they forced my elderly mother to sign unexplained documents and they attempted to transfer her to another facility with a two hour notice to pick up her belongings because they couldn’t be taken to the new facility. She was given a 30 day notice on 12/6 and they are trying to move her out and further away from her family with zero notice. We had not even had an opportunity to vet the facility. The administrator violated HIPPA telling us no authorized privacy information, threatened my mother, harassed her and proceeded to call my mother trash. There is so much more to this story but that will be heard at a later day. Until then, please do not leave your loved one at this facility. The staff refuse to provide any information regarding the management company or ownersThe facility was fined in 2024 for mismanagement of narcotics regarding a dying patient as well as lack of skills proficiencies for the medical and ancillary staff. They are extremely inattentive to the residents and impose “rules” when it is convenient for the staff. I had to give them one start in order to post. Centers for Medicare and Medicaid gives them the lowest possible score before they lose their accreditation making it impossible to take Medicare or Medicaid insurance.

My mom has been at Landmark for over a year now and we love the place. A family has to give nursing home a chance to get to know their new patient & family. Also the nursing homes have their own agendas that the absolutely have to go be. They are always short staffed. So keep in mind when one is criticizing the staff just remember how many people the nurses & their nurses aids are working and they are taking care of your loved one not you.

This place is ridiculous. I was going to go there for rehabilitation, and when I got there I said no way I am staying here. I watched how they were treating the residents there, and they were cruel. I asked to get water and they said “if the machine works”. There’s no updates, there wasn’t even any outlets in my room. When I was leaving I was in one of the facilities wheel chairs waiting for my ride and they yelled at me and said I needed to get out of their wheelchair. They pushed me down the hallway in the wheelchair and told me to pick my damn feet up and we’re getting very angry at me. Nurses were arguing with eachother across the facility. The person who picked me up witnessed a lot of this. We will be filing a complaint with the state of Oklahoma. This isnt how you treat people. I wanted to stay to get rehabilitation, but they just were not kind and I had a bad feeling about the place. Once I told them I wanted to leave, everything changed. This place needs to be reported, and just shut down. It’s obvious the only residents they can hold are the ones who cannot speak up for themselves. All about money.

my mother was there for rehab when i went to see her the nurse met me and said she was the worst person. when i got to her room she was naked and wrapped in her sheets. i asked her what was going on he told her to take her clothes off for a shower then never came back. i asked where’s your phone she told me that the nurse locked it. he said she was not allowed to have it i asked why she was bruised she told me he put his hands on her. he said his patients did what he said or he would make them do it. more than one other complaint that i was told about. he assaulted my mother and stole her phone. in my opinion he needs to be arrested.

If you want to send your family somewhere that will keep you worried about your family, THIS IS THE SPOT. If you want to be UNSURE there will be staff there to care for your family, this is the place. If you want to have NO communication between the staff, especially the social worker that is supposed to be there for your loved one this again hits the mark. These people are only worried about the money not a care in the world for the actual patience. A formal complaint will be made with BBB as well as other social platforms.
