This review has taken months to get in. I was admitted on 5/12/25 after issues from a left TKA. My first 2-3 days was difficult however the staff were totally awesome and made me feel much more relaxed as the days went on. I had one incident with a staff member that was handled with professionalism. My room needed changed due to a medical condition and made the rest of my time here very positive. I did have two issues. One, I was told I was getting an air mattress that never came and had a pretty painful bedsores. The second was an error with medication as some dosages changed, however, this one was resolved thanks to my advocates. Now the best part - the PT dept was phenomenal!!! Lisa, Susan, Joe, and the gang were amazing. Personally, i thought I would never walk again. The dedication these folks gave, their expertise and professionalism was second to none!!! I really can't say enough about this group of men and women. It was a very sad day when I was discharged. I promised them I would walk back in to see them to show them how far I came. That day is still yet to come, but getting closer and closer. I recommend this facility to anyone especially those needing PT and rehab. Thank you to the staff and administration for a job well done.

The skilled nursing team is attentive and professional, and they really seem to understand the individual needs of each resident. One standout feature is their in-house dialysis service, which makes a huge difference for those who need regular treatments—it's convenient and reduces the stress of having to travel for appointments. Additionally, the therapy services are excellent. The therapists are knowledgeable and work closely with patients to develop personalized recovery plans. It’s clear they care about helping individuals regain their strength and independence.

The facility itself doesn't deserve any stars. Most of the staff doesn't deserve any stars. They bring in people off the street on weekends because they don't want to pay their own staff. You never know what bum you are going to get from the so called "agency". Call lights take a minimum of 65 minutes each time. I've heard staff saying things that someone in that profession shouldn't be saying. The supervisors just sit at the desk looking at things. Only two supervisors do anything Tom and Nicole. The rest do absolutely nothing. Just tonight the night supervisor had to pass out meds and that was a total disaster and waited over an hour for pain meds until I got up myself and found someone. The Physical Therapy crew is great. All of them do a really good job and they actually care. The nurses that care are Cindy and Lynn they are great as well and they both care along with maybe 1 or 2 more. I've been here for a month due to an injury and I'm young enough to tell you everything wrong with the place and I can see people who care and the ones that don't. I would not put a single loved one here! All the 5* ratings are from people that work here. Please look somewhere else.

Please look elsewhere for a good home for your loved ones. This place is absolutely disgusting. The director of administration, as he called himself, has anger issues. He yelled at a staff over a spill I noticed, but I believe it was a leak from the air conditioner. As the other air conditioner had towels under the vent. The building doesn't have central air at all. Also, they have a community shower for the residents. I spoke to a few residents, and they mentioned that they are lucky to get a shower once a week at times. There's not many private rooms in the building. They have 2 beds in a majority of the rooms. I'm sure they break HIPPA laws since most rooms have 2 ppl in them. There's no privacy for the residents. I beg you to look elsewhere. I know we are.

My father spent 2 months at Transitions. We were so impressed with the therapy dept. Food was good too! He's now home and doing great thanks to Transitions!
