DO NOT USE THS FACILITY! DANGEROUSLY INCOMPETANT! I was there for approximately 6 weeks ending the last week in July 2025. Two weeks into the stay I got COVID as a new patient was brought in from hospital and there was inadequate quarantine. 4 weeks later i got severe diahrrea (completely liquid); immodium and pwerful anti- diahrreal meds didn't touch it. An inadequate stomach x-ray "revealed" a blockage or so the doctor, who never personally saw me, relayed through the nurses - they gave me myralax twice daily. They said they took stool samples though two were left on the TV counter in my room; CNAs reported to me none went out to the labs. They claimed I wasn't dehydrated though 9 days of 6-8 liquid stools per day, chaffed lips, and dry mouth and low urine output said different. I wanted IV fluids; this was denied. Some CNAs got sick of changing me and told me they'd check on me every 2 hours and not to bother them in between. Concientious CNAs could tell it was an infection by odor and consistency and encouraged me to call 911. I resisTed at first, trusting the nurses, but as each day passed with me getting worse i finally called though the nurses stuck to the "blockage" theory and tried to talk me out of it. At the hospital, CAT scan revealed no blockage, blood work confirmed dehydration and elecrolyte depletion, and stool samples revealed c-diff. Also, I'm diabetic but they never checked blood sugar. Was told I was on a low-carb diet yet served large plate of spaghetti and 2 slices of garlic bread. Checked vitalls only twice: arrival day and departure day. Was given 4 showers my entire stay. Supposed to see the doctor 3x a week; saw him a total of 3 times the entire stay. Discontinued my bipolar medication and had a manic episode - first time in over 20 years. Those staff that cared were actively looking for other jobs as they "don't want blood on our hands." Frequently sat in soiled briefs for hours which made my bedsore much worse. The only good thing I can say is the outstanding PT/OT department. The facility had the nerve calling me at the hospital asking when i was returning; I told them I don't feel safe with them and to not call again.

I wouldn't send my worse enemy to this septic tank of a facility. My brother was there for 12 days, they left him laying in bed with a diaper on. Didn't do anything requested per the Dr from the hospital. Diabetic diet, intense therapy. Never got any of those things. They would leave him in a souled diaper for 2 hours or longer. After 11 days of being there, the nurse finally checked his blood sugar and it was 457 and said "oh that's not bad." Seriously? I could right a 10 page complaint about this facility. But, I won't. If you love your people, do NOT send them to this facility.

My best friend has only been there 3 weeks . Every time I go to visit her,which is everyday, her colostomy bag was leaking on her and her bedding. I had to go to the store and buy sheets and a mattress pad for the bed they have her on. If you care about your loved ones, DO NOT LET THEM COME TO THIS PLACE!

Oak View is no where near a 4 star facility! Oak View on Kingsley Ave, Orange Park, Fl. is very unprofessional from administration to the nursing staff. Due to the lack of care and attentiveness they have caused my family member additional medical problems. They didn't follow doctors orders. No diabetic diet. Sugar was 457, They didn't start Physical Therapy resulted with a blood clot, Didn't rotate him in bed ended up with a bed sore. was a fall risk patient, had no bed rails. The list goes on. When choosing a facility for a loved one do not choose Oak View! Oak View did not meet my expectations.

I’m very disappointed in this facility. It’s located in beautiful orange Park. But however this facility and staff are very unsatisfactory. My brother was sent there for 3 hours a day of extreme rehabilitation that he only got 30 minutes 3 times in 8 days of being there. The Dr. didn’t put him on his diabetic diet nor none of the staff was checking his blood sugar levels. On the 7th day we ask what has his blood sugar levels has been. The nurse, Oh no we haven’t been checking that. Well we asked them too on the 8th day his blood sugar was 457 my brother could have slipped into a diabetic coma or worse. The Staff here are very lazy it took them over 3 hours to change my brothers diaper and over 2 hours multiple times leaving him in his soiled diaper. Then they get an attitude when you ask them for their assistance. They took him to rehab and after they just pushed him out into the hallway and expected him to sit in his wheelchair until the shift changed. He had to sit in his wheelchair for 2 hours one time because the staff there wants to push the work off to the next shift. He was threatened by another patient that shared a room. The nurses thought that was funny. This facility needs new management . David the manager has a real bad attitude and bad outlook on patients there . so I see why the staff acts so unprofessional.
